Worker's Compensation

Although Trust plans do not provide worker’s compensation benefits, there are several provisions in our policies that affect you. If you have questions about filing a worker’s compensation claim, you should contact your worker’s compensation insurer for assistance.
Health
Our health policies say that you must notify us immediately whenever a covered employee suffers a work-related illness or injury or files a claim for worker’s compensation benefits. (See Employer’s Duty to Furnish Information in Section 1 of the policy.)
When you complete the Employer's First Report of Injury or Disease form, please make a copy and mail it to:

If you prefer, you can give us the information by phone. Just ask for one of our worker’s compensation recovery representatives when you call.
The information you provide is important because none of our health plans cover services that are eligible for worker’s compensation benefits. (See General Exclusions in Section 5 of the policy.) When you notify us of the work injury or illness, we can take steps to ensure that your group health plan does not pay for expenses that are the responsibility of your worker’s compensation insurer.
Long term disability
Our long term disability (LTD) policy says that we will adjust a disabled employee’s gross benefit by any Other Income Benefits the employee is eligible to receive, including worker’s compensation benefits. That means that we will reduce a disabled employee’s LTD benefits by any wage-related worker’s compensation benefits the employee receives. When you inform us about work-related injuries, we can properly calculate and timely pay LTD benefits. If it appears the employee will be out for a period of time equal to or greater than the elimination period in the district’s plan, she/he should submit an application for LTD benefits even if they are receiving worker’s compensation benefits. Please pay special attention to Question 9 on the Group Long Term Disability Claim Form, Employer Information Section.
Long term care
Our group long term care plan does not pay for expenses that are eligible for payment under any worker’s compensation payment program established by law.
